If you visit KNO Medisch Centrum for a new complaint, you will need a referral letter from your GP. Sometimes the GP will forward the letter digitally and sometimes you will receive it with the request to bring it to the outpatient clinic.
In addition, you should always bring:
- The health insurance card from your health insurer.
- Your appointment card.
- A valid proof of identity (passport, driver’s license, identity card or residence permit).
- List of medication you are taking. Or the medicines themselves.
